You may print these forms and fill them out then send by mail or by fax, a brochure link is located at bottom of the page, or you may submit
it now. We will immediately acknowledge your reservation or we will contact you if the date and time you have selected are already booked
and seek your instructions as to how to proceed. You may check the open dates page if you wish we try to update it accordingly.

Please be sure to sign the authorization, your wedding date will not be processed without your signature. We will also need a
copy of your Driver License and a copy of the credit card showing the name on the front of your card.

Terms & Conditions

Officiating Fees

A non-refundable earnest fee of $100 is required at the time you reserve our services. This fee will be applied towards your total due.
The balance is due 60 days before the wedding date.*
Reservations made with less than 60 days remaining until the wedding date
must be paid in full by Cash, certified funds, Visa or Master Card.
No personal checks please!

Terms

Refunds: Cancellations that are received more than 60 days before the reserved date, shall receive their money back, less a $100.00 earnest fee for
processing your information. Cancellations received with less than 60 days until the reserved date shall not be refunded any monies. We can however
change your date if necessary as long as we have no other weddings booked that date.
Please call us to confirm your new date.
Refusal: We reserve to right to refuse any request for a reservation and we further reserve the right to refuse to perform any service that we are not
comfortable with, even though a reservation was accepted. If a wedding service that has been reserved is not performed, then a refund shall be due in the
same form received.

Travel & Expenses

Any travel of outside of Carteret County N.C. Shall be charged an additional 40 cents per mile.
All air travel, cabs or rental cars, meals, hotels, and other such necessities reserved, shall be at the expense of the client and must be booked well in
advance of the wedding event. We must receive written confirmation of these booked necessities 60 days prior to wedding date.
